Local health and care organisations have launched a new website, setting out the work they are doing together to improve care and transform services.
The website belongs to the Norfolk and Waveney Sustainability and Transformation Partnership (STP), which consists of around 13 local NHS and county council organisations that either provide care or support or are responsible for commissioning them.
Together the partners are working to improve services, make scarce resources meet demand, find better ways of providing services, help people prevent ill health and help people manage their health and care better.
The independent chair of the Norfolk and Waveney STP, Patricia Hewitt, said: “I hope that our new website will make it even easier for patients, service users, carers and the public to share their views and experience.”
The front page of the website features an interactive map showing a few of the successful improvements that partners have put in place.
The work of the STP is divided into three sections:
- “Healthier Communities” - working to empower local people to live healthier lives and prevent ill health
- “Better Care for People” - finding better ways of providing services, such as diabetes, cancer or mental health
- “System Transformation” - this section talks about things like using technology and digital solutions, our workforce and the STP demand and capacity review
